We are creating a
Memory Book
for All Alumni of
the
San Gabriel Classes of '57 and '56
The Memory Book will include all classmates
who wish to participate, whether you are able to attend the reunion or not. The sample above shows how each alumni will be shown
first in your original class photo
and then in a current photo.
The last day I can receive
your photo and page information
is Monday, September 10th...

to be included in the book!
Please complete this form and include your photo at the time you make your reservation.
The forms follow which can be downloaded and mailed along with a photograph
of you. We can accept color or black and white photos...the photos will be cropped and adjusted as best possible so that each of the images is similar in size to your original graduate photo in the annual.
The photos do not have to be of you alone...I can crop your face from a group photo...but I do prefer an original photo,
I will take good care
of it and mail it back to you.
If you are not able to attend the weekend celebrations,
all the more reason
we would want to have
your information for our Memory Book!!!

To return the form by e-mail

then paste this information from your clip board (ctrl+V)
to your new document where you will be able to insert your own text
and fill out the form, please use a different color for your test to help

The San Gabriel Country Club (old uniforms, club sweaters, jackets,photos, etc.) please let us know so that we can plan how to get them
together for the presentation.
The Memory Book will be given to every alumni who attends the weekend celebration. If you wish to have a Memory Book mailed to you please provide us with your name, complete address, and telephone or e-mail information and include your check for $15.00 (the cost of the book,
